  • Response Time: Submissions are reviewed daily during business hours and responded to within 1-2 business days.

  • Medication Refills: For refills, please contact your pharmacy first. If needed, you can request refills through this form or by calling us.

  • Appointment Changes: For cancellations or changes within 48 hours of your appointment, please call us directly. If outside of business hours, please leave a voicemail.

  • Urgent Requests: For urgent matters, call our office; after hours, you can leave a voicemail, and we’ll respond as quickly as possible the next business day. In case of an emergency, contact 911.​
